Gapps:
Use GAPPS Release 3.3. You can download GAPPS 3.3 Full package or Standard package (Recommend for Nexus 4 Users due System Storage Size).
Root:
Now ROOT is integrated with Privacy Guard and comes enabled by default.
BEFORE PROCEED, DO A FULL BACKUP OF YOUR DEVICE!
Install instructions:
First install:
Put ROM and GAPPS 3.3 zips in Internal Storage, go to recovery, wipe System, Dalvik and Cache.
Flash ROM + GAPPS and reboot.
To avoid issues, I recommend wipe Data too but you can try a dirty flash.
ROM is already rooted but you can download from Play Store or flash SuperSU too (link below).
Update install from R9.x:
Put ROM in Internal Storage, go to Recovery and flash ROM.
Update install from R5.5/R6.x/R7.x/R8.x:
Put ROM in Internal Storage, go to Recovery and flash ROM.
If you get com.android.phone FC, unfortunately you will need to do a clean install. Sorry!
Update install from R5.4 or minor:
Put ROM + GAPPS 3.3 in Internal Storage, go to Recovery and flash ROM and GAPPS R3.3.

If you prefer SuperSU over integrated ROOT, completely disable root access in Settings / Developer options, then flash SuperSU.
Franco Kernel:
If you want to use Franco Kernel, use the one on ROM Download Page.
DO NOT FLASH FRANCO KERNEL OR YOU WILL GET BOOTLOOP.
This happens due changes in Sepolicy on my RAMDISK for Theme engine and FRANCO KERNEL does't comes with this change.
Kernels with ANYKERNEL install method works fine.
